Register for Online Services

Once you are fully registered with the practice you will be able to access the following online services:

> Book/Cancel an appointment
> Request repeat prescriptions/medication
> Update your personal information
> View your Medical Records
> View your test results
 

Do not book for smear, dressing and wound care, travel immunisations, antenatal online.

How do I register for online services?

To register for online services you will need to complete a Patient Access Application form and read the Patient Information Leaflet. You will then need to visit the practice with photographic ID and reception will then issue you with a username and password. This is so that we can verify your identity and protect your confidentiality.

Form A : For self-online account (aged 16 and above)

Form B : Access for children, parents and guardians – Proxy access

Proxy access refers to access to online services by somebody acting on behalf of the patient and usually with the patient’s consent. The proxy does not have to be a registered patient at the practice.

myGP

Download our new appointment bookings app to your smartphone for free. myGP was developed to make access to healthcare easier and faster for patients. Once downloaded, simply log-in with your date of birth and mobile number and add the people you care for, allowing you to book and cancel appointments for them. It’s really that easy! Access your medical records, test results and request repeat medications through myGP using the online PIN received from the reception.

  • Important: Telephone System Fault

    Dear patient

    We would like to inform you that there has been a fault identified by BT with our telephone system including Automated System.

    This has been reported on 04.11.19, which was investigated by our telephone system provider and BT. They identified the fault and confirmed that this was resolved on 08.11.19.

    Unfortunately, we found out that patients still experiencing difficulties reaching us and calls were dropped during the weekend. We have raised another log to resolve the issues as soon as possible.

    If you have difficulties reaching out of hours please call 111 to get non urgent advice, for urgent health support please call 999.

    We have started the process to replace our current system and wish to complete the process as soon as possible.

    Apologies for any inconvenience.

     

  • Medical Certificate/Sick Note

    You only need a doctor’s certificate if you are sick for more than 7 days. Until 7 days you can use a self-certificate available on HMRC webpage.

    For a doctor’s certificate of more than 7 days, submit an online request.

    If you are interested in joining our patient participation group, Please contact the reception. The next patient group meetings are on 7th Nov 2019, 9th January 2020 and 5th March 2020 at 6.15pm.

    If you need support to stop smoking, please book an appointment with our smoking Cessation Advisor on Mondays between 3.30pm – 4.45pm.

    Citizen Advice Bureau Service available on Wednesdays between 2.00pm - 4.30pm.. Please speak to the reception.

    We offer travel vaccinations for our registered and non registered patients. Hepatitis A and B, Typhoid, Meningitis ACWY, Meningitis B and Yellow Fever.
