Collaborating and meetings with wider community health professionals (MDT’s in Diabetes, asthma, Copd, psychology, Integrated care meetings including palliative care)
We are involving our stakeholders by collaborating with Patient participation group, responding to patient’s feedback and concerns including patient’s complaints.
